Items Needed to Obtain a Passport Book or Card

*Allow a full 6 weeks to process your passport application.

•  Completed Passport Application- applications are available in the Auditor's office or online at Travel.State.Gov
•  One 2x2 Passport picture - available at a local vendor such as the Adair County Free Press
•  Certified Copy of Birth Certificate or Original Citizenship Certificate                                            

For applicants 18 years of age or older:

• Must appear in person to apply with Drivers License or other government PHOTO ID
• Fee: $110.00 - must be check or money order payable to US Department of State.
• Fee: $25.00 - must be cash or check payable to Adair County Recorder.

      Total Passport Fee:  $135.00 (Passport is valid for 10 years)

For applicants 16 or 17 years of age:

• Must appear in person with their driver's license and checks signed by a parent. One of the parents must appear with their driver's license to give permission for child to apply for passport.
• Fee: $110.00 - must be check or money order payable to US Department of State.
• Fee: $25.00 - must be cash or check payable to Adair County Recorder.

      Total Passport Fee:  $135.00 (Passport is valid for 10 years)

 For applicants age 15 or under:

• Must appear in person with Both parents or legal guardian. Both parents must sign the application and have valid photo identification. Ask for details if one of the parents is unable to sign the application.
• Fee: $80.00 - must be parent's check payable to US Department of State.
• Fee: $25.00 - must be cash or check payable to Adair County Recorder.

      Total Passport Fee:  $105.00 (Passport is valid for 5 years)

All applicants must know exact dates of parent's births, whether living or deceased. Applications will not be processed without this information.

Passport "cards" are also available at a reduced fee of $55.00 for US Citizens who frequently travel across the border by land to Canada or Mexico. These cannot be used for air travel. The same application requirements apply.

If you are traveling in less than 6 weeks, you will need to expedite your application.  An additional $60.00, plus fees for overnight envelopes will apply.  You will receive your passport in 2-3 weeks.  Ask for further details.
